Essential Oversight

Comprehensive Care
$1,495/ Per Month
Best for families managing active complexity who need consistent coordination. Not yet at crisis level, but beyond stable.
For families navigating ongoing complexity, this tier goes beyond monitoring—you’re coordinating, communicating, and making decisions. It keeps you informed and ahead, so nothing important falls through the cracks.
What is included
- Bi-weekly in-home nursing assessments, two per month.
- Medication management and review
- Provider coordination and communication, up to 2 hours per month
- Appointment accompaniment available up to once per month as needed
- Monthly care plan review and update
- Written family progress updates
- Distance family briefings by phone or video
- Priority phone and email access
Additional coordination beyond 2 hours per month is available at 75 per hour.
Premium Concierge

Best for high-acuity, rapidly evolving, or end-of-life situations when Comprehensive Care is no longer sufficient.
For high-stakes situations with little margin for error, this tier offers a deeper level of care—a dedicated nurse embedded with your family, responding urgently and supporting you when it matters most.

- Care coordination up to 4 hours per month
- Appointment accompaniment up to twice per month
